令和6年度医療介護連携研修会　事例発表

介護福祉士会より依頼のお声を頂き、備北地域における「医療介護連携研修会」にて当施設から統括主任山田が事例発表を行いました。

この研修会のテーマが「チームケアのための介護職と医療職に求められる役割等について」となっており、施設ケアにおいて「最期は施設を選ぶのか、病院を選ぶのか」について発表をしました。
当施設は広島県北では唯一の介護付き有料老人ホームであり、可能な限り看取りも行える貴重な存在であります。

しかしながら地域の協力医の理解と協力なくしては行えません。本人様、ご家族が最期まで安心して生活して頂くために何ができるか？を考え、自分の役割を全うしていく事が重要であります。
この発表が施設の取り組みについて知って頂く機会となりました。

At the request of the Association of Care Workers, our general manager, Mr. Yamada, presented a case study at the “Workshop on Medical Care Cooperation” in the Bihoku region.

The theme of the workshop was “Roles required of caregivers and medical professionals for team care,” and he gave a presentation on the question of whether to choose a facility or a hospital for the final stage of care at a facility.
Our facility is the only private-pay facility with nursing care in the northern part of Hiroshima Prefecture, and it is valuable in that it is able to provide end-of-life care whenever possible.

However, we cannot do so without the understanding and cooperation of local doctors. What can we do to ensure that the patients and their families can live in peace until the end of their lives? It is important for us to think about what we can do and fulfill our roles in order for the patients and their families to live their lives with peace of mind until the end of their lives.

This presentation was an opportunity to let people know about the efforts of the facility.
